  1. We receive your short referral.
  2. We read the general support request without asking for the participant’s private records.
  3. Ben, Tristan or Logan contacts you in the way you prefer.
Use the participant’s correct registered name.Enter the participant’s full legal name exactly as registered with the NDIS. Please do not include their NDIS number, health records, plan or identity documents in this short referral.
